Bear Hill Rehab Receives CARF Re-Certification

Bear Hill Rehabilitation and Nursing Center received its three-year accreditation from the Commission on Accreditation of Rehabilitation Facilities (CARF) in May 2016. CARF was founded in 1966 as an international, non-profit organization whose mission is to provide accreditation standards and surveyors for organizations working in the health and human services field worldwide, with a base in traditional facilities and institutional settings. Accreditation covers the following areas, among others:

  • Aging Services

  • Behavioral Health

  • Opioid Treatment

  • Business and Services Management Networks

  • Child and Youth Services

  • Employment and Community Services

  • One-Stop Career Centers

  • Vision Rehabilitation Services

  • Medical Rehabilitation

  • DMEPOS (Durable Medical Equipment, Prosthetics, Orthotics, and Supplies)

Person Centered Care is based on patient/resident preferences, shared decision making and initiation for self care management whenever possible.

Assessments, interventions and plans of care address medical, clinical and behavioral needs and reflect the individual’s ability to contribute to making their own healthcare decisions.

Person Centered Care is a process for planning and supporting the patient/resident receiving services that builds upon the individual’s capacity to engage in activities that promote community life and that honors preferences, choices and abilities. The person centered care process involves families, friends, and professionals as the individual desires or requests.
